This was a regular occurance in the Parramatta river in Sydney growing up as a kid, especially around all the industrial areas where heavy toxins were present in the water from waterfront factories such as Union Carbide and Dulux Paints.

Saddleback was just one of the deformities we would see...... twin tails and fins were also another regular occurance.
